Schedule an Appointment
How to Schedule
A referral is not necessary for an appointment. Clients, parents, and legal guardians can call any of our offices to schedule an initial assessment. You can make an appointment by calling either office directly and selecting option 2. Make sure to take a look at the following instructions on how to prepare for your session prior to your arrival.
Initial Appointment
The first appointment is used to gather information from the client. The client and the clinician will use the information gathered in the assessment process to develop a plan for treatment. The initial assessment will last approximately one hour. Clients should arrive 30 minutes ahead of time to complete intake documentation.
Payment is due at the time of service.
Preparing for an Appointment
Adult Clients
WHAT DO I NEED TO BRING TO THE FIRST APPOINTMENT?
Please bring your insurance card, your identification, and your co-payment (cash, check, or debit/credit/hsa). If you are not the primary insured, please be prepared to provide information about the primary insured person. Also, be prepared to tell us about any medications you are taking, who is prescribing them as well as your primary care provider information.
WHAT HAPPENS AT THE FIRST APPOINTMENT?
The first appointment is called the "assessment" in which we discuss and gather essential biological, psychological, and social presentation that is necessary for an accurate assessment of your condition. From this point, your therapist will complete an assessment and process information regarding the findings and possible treatment recommendations. We respectfully request you leave all children at home or with a caregiver as we need to focus entirely on you. During the first follow-up session after assessment, you and your clinician will develop a treatment plan.
Child / Adolescent Clients
WHAT DO I NEED TO BRING TO THE FIRST APPOINTMENT?
Parents, please bring your child's insurance card. If you are not the primary party or individual insured, please be prepared to provide information about the primary insured individual or party. Also, please be prepared to provide information regarding the client's primary care provider, a list of medications, and information regarding the client's prescribing providers.
WHAT HAPPENS AT THE FIRST APPOINTMENT?
Both the legal guardian and the child/adolescent client need to be present for the first session. The clinician will meet with both the child/adolescent client and the guardian during the assessment. This allows us to gather essential biological, psychological, and social presentation that is necessary for an accurate assessment of your child. Based on the preference of the clinician, the child and/or parent may be seperated for part of the session to assist in rapport building and gathering pertinent information. During the first session, the clinician will complete an assessment and process information regarding the findings of the assessment and possible treatment recommendations with the client and legal guardian. During the first follow up session after the assessment, the child/adolescent's clinician will meet with the client and his or her guardian to develop their treatment plan.

Insurance and Payments

Is PCH "In Network"?
We are "in Network" with several insurance panels to help make mental health services more affordable for our clients. Not all clinicians are paneled with all insurances which may affect availability when choosing a provider. At the time of referral we will schedule your appointment with an appropriate provider and contact your insurance company to confirm benefits such as deductibles, copays, or co-insurance. All subsuquent fees are due at time of service.

Payments
At PCH, keeping costs to a minimum is a priority.

If you do not have Medicaid or other insurance coverage, we will see you on a self-pay basis. If you do not have insurance coverage and cannot afford the self-pay fees, we will try to assist you in finding services that are government-supported so that you may receive needed treatment.
Appropriate forms of payment are Cash, Check, Credit or Debit Card (Visa/MasterCard). There is a $25 Returned Check Fee.
